The project aims at providing shared tools that put the individual at the center of decision-making about their life and the service and support they want or need. The person centered approach is a major factor for transforming public services and the personalization of social care for old people. It is a way to get most benefits of limited ressources and to improve the quality of life for old people.
We propose a collaborative platform shared with professionnals and individuals (including family and relatives) to define the person's goals, priorities and support needs. The core elements are :
- clarify expectations, understand the person's wishes and their story or history
- information on the ways for people to get support if they need it,
- review of possibilities taking into account the person's capabilities as well as the market or institutional opportunities,
- action planning
- and review process : making evidence of what is working and not working, change and commissioning decisions.
The platform will be sustained by a prior methodological approach and guide and all will be designed and tested in situ.
As the person is the major actor in the process, the human interface should be adapted to ageing people and attractive for the person, family and relatives. For professionals, mobility tools are required as well as interfaces with usual professionnal tools.
Specific security constraints will apply because most personal datas are strictly confidential.
